Penile or Foreskin Infection: Causes, Symptoms, Diagnosis & Treatment Guide
A penile or foreskin infection can cause redness, itching, swelling, pain, discharge, or discomfort while urinating. It may affect the head of the penis (glans), the foreskin, or both. Inflammation of the glans is called balanitis, and inflammation of both the glans and foreskin is called balanoposthitis.
Common causes include fungal or bacterial infection, poor hygiene, skin irritation, and some sexually transmitted infections (STIs). Treatment depends on the cause, so an accurate diagnosis matters.

What Is a Penile or Foreskin Infection?
It occurs when the skin and tissues of the glans or foreskin become inflamed or infected. It can develop suddenly or gradually, and it is more common in uncircumcised men because moisture and debris can collect under the foreskin.
Symptoms can include redness, swelling, itching, soreness, unpleasant odour, discharge, and difficulty retracting the foreskin. Some men also have burning during urination or pain during sex.
Mild irritation may settle with gentle hygiene, but persistent or recurrent symptoms should be checked by a qualified healthcare professional.
What Causes Penile or Foreskin Infection?
- Fungal infection. Candida, a type of yeast, can inflame the penis and foreskin. Risk is higher with diabetes, recent antibiotic use, or persistent moisture in the genital area.
- Bacterial infection. Bacteria can multiply under the foreskin when sweat, moisture, or debris builds up. This may cause redness, swelling, tenderness, discharge, or a bad smell.
- Poor hygiene. Sweat, oils, and dead skin cells trapped under the foreskin can lead to irritation and infection.
- Skin irritation or allergic reaction. Soaps, perfumes, shower gels, lubricants, condoms, and other products can irritate sensitive skin. In these cases, inflammation rather than infection may be the cause. Over-washing or scrubbing can also irritate the skin.
- Skin conditions. Eczema, psoriasis, and lichen sclerosus can cause recurring inflammation and may lead to a tight foreskin (phimosis).
- Sexually transmitted infections. Herpes, gonorrhoea, chlamydia, and syphilis can cause genital symptoms. If symptoms appear after unprotected sex, STI testing may be recommended.
- Diabetes and other health conditions. Repeated balanitis can be linked to poorly controlled diabetes or other underlying problems, so recurrent episodes should not be ignored.
Penile and Foreskin Infection Symptoms
- Redness or inflammation of the penis
- Swelling of the glans or foreskin
- Itching or burning
- Pain or tenderness
- White or yellowish discharge
- Unpleasant genital odour
- Dry, cracked, or irritated skin
- Burning or discomfort while urinating
- Pain during sexual activity
- Difficulty retracting the foreskin
- Small sores, blisters, or ulcers in some infections
Seek urgent medical care for severe swelling, inability to urinate, or a foreskin stuck behind the glans that cannot be brought forward (paraphimosis). This is a medical emergency.
How Is a Penile or Foreskin Infection Diagnosed?
Diagnosis usually starts with a medical history and physical examination. The doctor may ask about how long symptoms have lasted, hygiene habits, sexual history where relevant, previous infections, medications, and conditions such as diabetes.
Depending on the findings, tests may include:
- Swab tests to identify bacterial or fungal infection
- Urine tests
- STI tests
- Blood glucose tests if diabetes is suspected
- Further investigations for recurrent or unusual symptoms
Penile and Foreskin Infection Treatment
Treatment depends on whether the cause is fungal, bacterial, an STI, irritation, or another condition.
- Antifungal treatment: for a confirmed fungal infection, a doctor may prescribe an appropriate antifungal medicine.
- Antibiotics: bacterial infections may need antibiotics, chosen according to the suspected or confirmed infection.
- STI treatment: this depends on the specific infection. Sexual partners may also need evaluation and treatment.
- Managing irritation: avoid the triggering product and use gentle hygiene measures while the skin recovers.
- Recurrent or severe problems: repeated infections or persistent difficulty retracting the foreskin may need a
urologist’s evaluation. In selected cases, circumcision may be considered, especially with significant phimosis.
What to Expect During Recovery
With the right treatment, mild infections often improve within days to a couple of weeks, though this varies with the cause. Keep the area clean and dry, follow the prescribed course fully, and consider avoiding sex until symptoms clear or your doctor advises otherwise. Return for review if symptoms do not improve or come back.
How to Prevent Foreskin Infection
- Wash the penis gently with water and mild, non-irritating products.
- If the foreskin retracts comfortably, gently clean underneath it. Never force it back.
- Dry the area well after bathing.
- Avoid harsh soaps, perfumed products, and unnecessary chemicals.
- Change out of sweaty or wet clothing promptly.
- Practise safer sex and use condoms where appropriate.
- Keep diabetes well controlled if you have it.
- Avoid self-medicating with antibiotics or steroid creams without medical advice.
When Should You See a Doctor?
Consult a doctor if symptoms are severe, persistent, recurrent, or worsening despite basic hygiene. Evaluation is especially important if you have:
- Significant swelling
- Pus or unusual discharge
- Genital ulcers or blisters
- Severe pain or fever
- Difficulty urinating
- Difficulty retracting or replacing the foreskin
If you develop genital symptoms after unprotected sexual contact, ask a healthcare professional about STI testing.

Frequently Asked Questions
Many penile infections are easily treated, but the cause must be identified. Persistent or severe infections can lead to complications, so symptoms should not be ignored.
It depends on the cause. Fungal infections may need antifungal medication, bacterial infections may need antibiotics, and irritation may settle once the trigger is removed.
Yes. Sweat, oils, and dead skin cells collecting under the foreskin can cause inflammation and infection. Gentle, regular cleaning helps reduce the risk.
Yes. Poorly controlled diabetes increases the risk of genital infections, especially recurrent fungal infections. Repeated infections may warrant a blood glucose check.
Not always. Some causes, such as candida infection or STIs, can pass between partners, while balanitis caused by irritation or hygiene is not contagious. A doctor can advise on your situation.
A urologist may be appropriate for recurrent infections, persistent foreskin problems, phimosis, significant swelling, or symptoms that do not improve with initial treatment.
